Note: Park Entrance Fee: Brant Conservation Area charges a reduced entrance fee of $4.50 per Participant, $5.50 per Spectator ($1.75 ages 6-14, children under 6 free) which includes parking and all day use of the conservation facilities.

Technical Tees are guaranteed if ordered before May 30th.
*Slight change for the 25k route, the 3k loop will be run at the begininng of the race, instead of previous years at the end.

In consideration of the acceptance of my application and for the permission to participate as an entrant or competitor in the "PHLOX RUN" to be held on June 10, 2012, I, for myself, my heirs, executors, administrators and assigns HEREBY RELEASE, WAIVE AND FOREVER DIS CHARGE The County of Brant, The Corporation of the City of Brantford, The Grand River Conservation Authority (GRCA), Runner's Den (Paris), Runner's Den Ltd. and all other associations, sanctioning bodies and sponsoring companies and all their respective agents, officials, servants, volunteers, contractors, representatives, successors, and assigns OF ALL AND FROM ALL claims, demands, damages, costs, expenses, actions and causes of actions, whether in law or equity, in respect of death, injury, loss or damage to my personal property HOWSOEVER CAUSED, arising or to arise by reason of my participation in the said event, whether as a spectator, participant, competitor or otherwise; whether prior to, during or subsequent to the event AND NOTWITHSTANDING the same may have been contributed to or occasioned by the negligence of any of the aforesaid. I FURTHER HEREBY UNDERTAKE TO SAVE HARMLESS and AGREE TO INDEMNIFY all of the aforesaid from and against any and all liability in-curred by any or all of them arising as a result of, or in any way con nected with my participation in the said event.
ATTESTATION
I attest and verify that I am fully aware of the physical risk of injury or death in participating in this event and voluntarily agree to accept full re-sponsibility and legal liability for same, and I am physically fit and have trained sufficiently for this event and have no medical condition that might be worsened by physical activity. I am also aware that the weather conditions on June 10, 2012 may be unpredictable thus posing increased physical risk. This release extends to all claims unforseen, known or un-known.
Further, I grant permission to the Phlox Run, and any media covering this year's race, and the authorized agents, contractors and representa-tives of each, to use my name and likeness in any photographs, video-tapes, motions pictures, recordings or any other records of my participation in this event for any purpose.

BY SUBMITTING THIS ENTRY I ACKNOWLEDGE HAVING READ, UNDERSTOOD AND AGREED TO THE ABOVE WAIVER, RELEASE AND INDEMNITY I WARRANT that I am physically fit to participate in this event.

I ATTEST AND VERIFY THAT I AM 18 YEARS OF AGE OR THAT I AM SIGNING AS THE LEGAL GUARDIAN ON BEHALF OF AN EVENT PARTICIPANT UNDER THE AGE OF 18 YEARS.

Agreeing to this waiver and registering online for this race by me, for myself or as legal guardian of an event participant, in an electronic format authorizes you to accept this waiver as signed by me.
